Since 1996, CareConnectMD (formerly Gerinet Medical Associates) has been providing personalized and compassionate medical care for our frail and medically complex patients in skilled nursing and long-term care facilities. We understand transitional times can be challenging after surgery, injury, or serious illness. Oftentimes, post-acute care, or care provided to patients after they are discharged from the hospital, is disjointed and disconnected from the rest of the health care system resulting in poor coordination of care, unnecessary emergency room visits, and poor patient outcomes.

Here at CareConnectMD, we bridge the gaps in care as our patients transition from inpatient to post-acute settings, including going home. Our highest priority is to ensure our patients receive the right care at the right time, through improved communication, better coordination, and compassionate care. Our 24 years of experience managing care for institutionalized patients has helped us design an integrated care delivery model that improves patient care, reduces overall system costs, and improves patient, family, and facility satisfaction.

Our value-based, connected care experience has helped us successfully navigate hundreds of thousands of our patients in California, Nevada, and Arizona through the healthcare system, dramatically improving their quality of care and their quality of life. We currently serve over 110,000 patients at more than 360 SNF's and long-term care facilities in San Diego, Orange, Riverside, San Bernardino, and Los Angeles counties, as well as Tucson, AZ. In 2018, we expanded our coverage to include supportive care and telehealth services for Southern California and Arizona.
