Dr. Ken Kim, MD
Clinical Advisor
Dr. Ken Kim, MD is a Clinical Advisor for CareConnectMD at Home. He was the Chief Medical Officer with the Center for Medicare and Medicaid Innovation (CMMI) from July 2018 to July 2019, where he advised on policy, clinical model development, quality improvement, and model performance review. He was also the Chief Medical Officer at Alignment Healthcare LLC, from 2013 to 2018 and responsible for creating, developing, and executing Alignment Healthcare's proven clinical health model, while driving future state innovation. Board-certified in Internal Medicine, Dr. Kim joined CareMore in 1999 as a hospitalist. He became a Regional Medical Officer at CareMore in 2008 and was responsible for clinical operations in Downey, Lakewood, and Long Beach as well as all chronic disease management programs and mental health programs. Appointed Chief Medical Officer of CareMore from 2010-2013, Dr. Kim expanded and replicated the clinical model from Southern California to San Jose, Las Vegas, Modesto, Phoenix, Tucson, New York, and Richmond. During his tenure as CMO at CareMore, membership grew from 30,000 to 70,000. Dr. Kim holds a B.A. and M.D. from Indiana University. He completed his internal medicine residency at Loma Linda University.
Dr. Liliane Idylle, DO
Senior Medical Officer
Dr. Liliane Idylle, DO is our Senior Medical Officer and leading our CareConnectMD at Home Program. Dr. Idylle is a board-certified Internal Medicine physician. She has a B.S degree in Biochemistry from UCLA, graduated from Des Moines University’s College of Osteopathic Medicine, and completed her residency training at LAC+USC Medical Center. Shortly after, she joined CareMore Anthem Health plan as an Extensivist physician focusing on highly complex, high-risk patients with chronic medical conditions. As an Extensivist, she provided compassionate patient care and followed patients from their arrival to ED, throughout their hospitalization, and as they transitioned to skilled nursing facilities. Once discharged, she provided outpatient follow-up at comprehensive care centers, closing the loop to ensure proper continuity of care. Well recognized by Caremore Clinical Leadership for her clinical acumen and leadership skills, Dr. Idylle was quickly promoted to Associate Regional Medical Officer for the North LA/OC region, where she was responsible for the coordination of care for 18,000 members. Dr. Idylle is passionate about patient care, particularly in the elderly and believes in providing the best care possible by aligning the patients’ goals of care by partnering with the medical team, patient, and their families. When she is not seeing patients, she enjoys spending time with her family, reading, baking, and traveling. As the Senior Medical Officer leading the expansion efforts of our DCE, she looks forward to continuing to be a patient’s advocate and helping patients and their families understand their complex disease process with the goal of improving patient outcome, family satisfaction while reducing overall system costs.
Angela Hewan, NP
Nurse Practitioner
Angela Hewan is a Nurse Practitioner who joined CareConnectMD as VP of Clinical Operations in August 2020. She is a graduate of UCLA and has been practicing since 1991. She has worked in long term care and hospice for the past 25 years. Prior to CareConnectMD, she worked at Seasons Hospice and Palliative Care as the National Director of Clinical Operations covering the Western United States. Her interests also include travelling and reading. When she is not seeing patients, she enjoys testing new recipes and trying to grow a vegetable garden. Her favorite part about working for CareConnectMD is the people and the impact that our providers have on the quality of life of our patients. Angela believes CareConnect is at the forefront of healthcare initiative, and that we could set the standard for palliative healthcare by which all other plans will be measured.
Marianne Plakas, FNP-BC
Family Nurse Practitioner
Marianne Plakas is a Family Nurse Practitioner for Los Angeles and Orange Counties. She has been practicing for nine years and has been with the company since April 2017. Prior to CareConnectMD, she worked at Sylmar Medical Center. Her interests include music and playing the piano. When she is not seeing patients, she enjoys spending time with family. Her favorite part about working for CareConnectMD is the flexibility of the job and the autonomy it offers all employees.
